Welcome to University Associates and thank you for considering us for your health care. When you call to make an office appointment please have your insurance card handy. You will be asked for that information. It is also important to have your insurance card with you at your first office visit and whenever changes have been made, since we will need an updated copy. 

In order to expedite your care and hopefully reduce the time you need to spend filling out forms when you come to the office, please fill out the following forms prior to your office visit. These can either be filled-out on line or downloaded to your computer and then either mailed to us or brought with you on your first office visit. The information on these forms will allow us to rapidly get to the substance of your problem and allow us to spend more time discussing your diagnosis, further evaluation and treatment alternatives with you. 

It is extremely important that you bring or have delivered to us any X-rays or tests that are pertinent to your diagnosis. Delays may result as we attempt to get the necessary information if it is not available when you are seen. In some cases we will need to reschedule the office appointment. You should also call the office of your referring physician and ask that they provide us with any information pertinent to confirming your diagnosis and rendering you care.
