Appointment Information

Office hours are 9 a.m. to 5 p.m., Monday through Friday.  However, individual members of University Surgical Associates conduct office hours at various times.  You may schedule an appointment by contacting the individual physician office. A list of office phone numbers are provided in this website under "Find a Physician". For general inquiries please call 401-272-1800. For questions regarding pediatric appointments please call 401-421-1939. 
 
Prior to your appointment, below are a few helpful hints which will faciliate your appointment:
  • Please arrive early to register and complete any forms that are required for your visit.
  • Some insurance companies require a referral from your Primary Care Physician. Please contact our office to ensure the referral is in place prior to your appointment.  If a referral is not receieved prior to your appointment you may be asked to sign a self referral form.
  • Copays are due at the time of service. For patients that are uninsured please ask to speak to the office manager prior to your appointment. The office manager can asssit you with financial arrangements.
